Data Service Center

For those investigators who do not have the time, staff or resources to collect and manage their data via a web-based subscription, PhDx maintains a data service center with a full array of mailing, collection and data entry services. 

PhDx can mail investigators paper copies of the case report forms needed for each patient for the life of the study. Once a case report form is filled out, it can be mailed or faxed back to PhDx for data entry.

PhDx will:

• Manage the collection of paper case report forms from physicians and patients
• Enter survey data into the database
• Perform patient tracking and follow-up post-procedure
• Send investigators monthly reports which include outcomes and past due case report forms
